transcripts, certification, documentation of Act 48 hours, three
reference letters, current clearances for PA Criminal Records, PA Child
Abuse, FBI Federal Criminal Record Check and PDE Sexual
Misconduct/Abuse Disclosure Release form(s), Act 24
Arrest/Conviction Arrest Report form. Information on these
