3. Three recent letters of recommendation dated within the last 2 years - At least 1 from a current or former supervisor

4. Detailed valid CA Speech Language Pathologist Credential - - Attach a printout with the detailed Document Number from the CTC website (Do not attach the certificate) or status letter from

credential analyst confirming credential recommendation by August 1, 2025

5. Transcripts (post-high school)
