The Hartford School District is seeking a Physical Therapist to work in several of our Hartford Schools and Programs with students who require Physical Therapy service to access their educational setting.The Hartford School District has a diverse team of related service providers that collaborate and support each other on a regular basis. Caseloads are reasonable and duties diverse. The OT/PT team meets on a monthly basis to support and cross train.

Job Requirements
- Provision of direct services as determined by students IEPs;- Provision of consultation services with the educational teams, families, related arts teachers, and paraprofessionals as indicated. This PT consultation may include but not limited to: - Interpretation of assessments and recommendations; - Explanation of the potential impact of developmental, medical and or sensorimotor impairments on educational performance; - Setting realistic expectations of students motor performance in school; - Selecting, modifying of adaptive seating as indicated; - Performing minor repairs of students equipment as needed; - If indicated, annually develop the students Evacuation Plan and provide training to all involved staff on evacuation procedures. - Provide family, equipment vendors, and outpatient providers with durable medical equipment recommendations to promote student success in the school environment - Educate school staff regarding medically relevant student information- The PT will work with students of all age ranges on gross motor skills, strength, balance, coordination, motor planning and/or sequencing. (PTs do not provide self regulation, self care and/or activities of daily living ADLs)- Participation in IEP meetings, progress meetings, special education evaluation meetings, and building/program based staff meetings as appropriate;- Administering and scoring assessments and writing evaluation reports for students in the special education evaluation process;- Developing annual IEP goals and objectives;- Develop a student schedule for physical therapy services that aligns with their physical needs, educational needs, and the services outlines on their IEP- Developing recommendations for school based accommodations and environmental modifications, including tools that promote student independence within the classroom setting;- Completing paperwork/documentation in EdDocVT and Medicaid; - Ordering, maintaining, storing and minor repair of required tools and/or school based equipment needed for students success in the school environment;- Train school staff on mobility related student protocols designed to address students personal needs and safety within the school environment- Assist with coordination of Extended School Year plans with teams and the Office of Special Education; - Conduct Physical Therapy screenings when requested by EST teams or special educators- Communicating with students medical providers as needed
