Occupational Therapist

Valid Kentucky Board Licensure for Occupational Therapy REPORTS TO: Director of Special Education PERFORMANCE STANDARDS: Conducts appropriate evaluations of children referred for possible special education services and prepares written reports of the evaluation conducted and the findings Participates in meetings as a member of the Admission and Release Committee (ARC) Provides information for and participates in the development of IEP and Annual Reviews of Progress Develops a treatment plan based on the evaluation results and IEP and provides related services on a direct or consultative basis Provides written progress reports of occupational therapy progress on the same schedule established by the local school district Provides written records for discontinuation of occupational therapy Collaborates with school personnel regarding occupational therapy services. Provides written daily notes in a format that meets district, state, federal and reimbursement needs and standards Collaborates with the Director of Special Education to plan for current and future occupational therapy services, including personnel, supplies, and equipment needs. Collaborate with the Director of Special Education to establish and review policies for the initiation, intervention and discontinuation of occupational therapy services in accordance with district, state, and federal regulations Provides education and In-service to school personnel and agencies related to school-based occupational therapy as requested by the school district Establishes, documents, and maintains a schedule of services provided and hours worked for billing and planning purposes Lifts, transfers, and positions children and equipment as necessary to provide occupational therapy Transports all supplies and equipment necessary to provide occupational therapy services Responsible for insuring records are filed appropriately and Medicaid is billed Performs related duties and other responsibilities as may be assigned by the Director of Special Education, Principal or the Superintendent
