What You'll Do: Effectively screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders. Develop individualized plans of care, utilizing evidence-based assessments and interventions. Monitor patient progress, adjusting treatment plans to ensure optimal outcomes. Evaluate and document treatment effectiveness, making necessary modifications and referrals. Provide consultation and counseling to patients, families, caregivers, and interdisciplinary team members. Communicate regularly with supervisors and healthcare team regarding patient progress and barriers. Prepare comprehensive discharge summaries and maintain detailed, compliant documentation. Participate in meetings, trainings, and ongoing professional development. Support clinical supervision of CFYs or student Speech-Language Pathologists. Ensure adherence to ASHA and State practice standards, ethics, and licensure requirements. Required Skills: Valid state license and ASHA certification as a Speech-Language Pathologist. Proven experience in diagnosis and treatment of communication and swallowing disorders. Strong assessment, clinical decision-making, and documentation skills. Excellent interpersonal skills to effectively communicate with patients, families, and team members. Ability to work independently and as part of a multidisciplinary team. Nice to Have Skills: Experience working in long-term care or institutional settings. Bilingual proficiency or additional language skills. Knowledge of telehealth platforms and digital documentation systems. Preferred Education and Experience: Master's degree in Speech-Language Pathology or equivalent. Prior experience in a clinical or long-term care environment preferred. Licensure and certification active and in good standing. Other Requirements: Ability to obtain necessary durable medical equipment (DME) and supplies. Willingness to support and supervise CFYs or student clinicians. Committed to practicing ethically and professionally per ASHA and state guidelines. Compensation for this rewarding role is highly competitive, with an hourly rate of $58.96.
